Community Approach & Impact

Angelica & Esteban

Angelica, Esteban, and their two young children were referred to the Cultural Broker Program for supportive services and ongoing case management. Upon intake, a comprehensive assessment was completed to identify the family’s immediate and long-term needs, barriers, and strengths in order to develop an appropriate service plan. Throughout their participation in the program, the Cultural Broker provided consistent guidance, advocacy, and coordination of services to ensure the family had access to necessary community and public resources. The family was successfully connected to and completed required parenting education classes, demonstrating commitment to strengthening parenting skills and family functioning.

In addition, the family received ongoing food assistance through regular food box distributions, which supported household stability and addressed food insecurity. The family was also provided with employment referrals and workforce-related resources to support increased financial stability and self-sufficiency. To further support the family’s basic needs, the Cultural Broker assisted the family with the reinstatement of Medi-Cal (Medical) benefits and CalWORKs, helping to restore access to healthcare coverage and public assistance to support overall family stability and well-being.

In recognition of the children’s needs and to promote positive family experiences, the family was connected to Toys for Tots, and the children received age-appropriate items during the holiday season. Additionally, Angelica, Esteban, and their children actively participated in multiple family-focused activities and community events, which promoted positive family engagement, strengthened family bonds, and supported healthy relationships.

The family demonstrated consistent participation, cooperation, and motivation throughout the duration of services. They maintained regular communication, followed through with referrals, and actively engaged in recommended supports. As a result of their commitment and successful completion of all program requirements, the family met and exceeded program expectations. Their progress and active engagement directly contributed to achieving case goals and supported the successful closure of their case.

Overall, the family demonstrated significant growth, improved stability, and positive outcomes as a result of services provided through the Cultural Broker Program.
